Frequently Asked Questions

How do I apply for KVP?  You must complete and submit an application.  You can print an application online by clicking on the Application Link.  Applications can also be obtained by calling the KVP Office.

 

How often can I use the program?  Due to the extreme high volume of applications, individuals can only use the program once.

 

How do you determine if I qualify?  Applications are screened by area Salvation Army and Community Action Centers to determine eligibility.  Eligibility is based on current federal poverty guidelines, which can be found under the KVP Guidelines Link.

 

How long does it take to receive an eye exam?  After an application is received and approved, individuals are assigned to see a doctor in their area for an eye exam.  Most eye exams are scheduled within 6 weeks after approval.  However there are few counties in the state with a very high volume of applicants and the waiting time in these counties can be much longer.  You can contact the KVP office to find out which counties currently have a waiting list.

 

Do I have to travel a long distance to see the Doctor?  No.   The KVP has participating doctors all over the state.  However, in order to ensure a shorter wait, we do ask each applicant to list at least 2 counties they would be willing to travel to.

 

How will I know if I have been approved?  Approved applicants will receive a letter from the KVP with information regarding their scheduled eye exam.  Applicants that are not approved will also receive a letter detailing the reason for their denial.

 

How much does the program cost?  The program is free to those who qualify.  However, because the Kentucky Vision Project is funded solely by private donations, a donation of  $25.00 is requested for those who receive glasses.
