MECHANICS OF

THE KENTUCKY VISION PROJECT

 

1.           Interested individuals obtain an application by contacting the Kentucky Vision Project or by visiting their local Salvation Army office or local Community Based Service Program.  Patients can only go thru the program once.

 

2.           The Salvation Army Community Based Services screens each application using the Federal guidelines of poverty level income and then marks and returns the applications to the KVP as either approved or not approved.

 

3.           Approved applicants are assigned to participating doctors in the county where they live. If there is no participating doctor in their county, they are requested to list two alternative counties where they can travel.

 

4.            Exams are scheduled in one of two ways:

 

a.      The patient will receive a notice from the Vision Project informing them to contact the doctor's office for an appointment. A date is stamped on both the notice and the transmittal form.  Patients are given at least two weeks to contact the doctors office to set up an appointment.  If appointments are not made within this time frame, a no show will be marked on the patient's file at the KVP office.

b.     The doctor's office staff will contact the patient to set up an appointment either by telephone or letter. 

 

Patient's should NOT CONTACT the doctor's office , unless informed to do so by the KVP office.  Failure to abide by these guidelines may result in ejection of the  program.

 

5.            All participating doctors donate the exams, however, if glasses are needed a $25 donation from the patient to the Kentucky Vision Project is requested.  The Kentucky Vision Project is a private funded organization and it depends on these donations to keep the program on-going.
